Human Services office in South Bend will be closed daily from 12:00 pm –
1:00 pm for lunch. There will be no nurse available in the office on Fridays. The office will be open & administrative staff available to assist the public. |

Maternity Support Services (MSS): comprehensive health assessment, education and support provided for pregnant women by a team of professionals including a nurse, registered dietician, and social worker. Maternity Case Management (MCM): ongoing health assessment, advocacy, education, resource coordination and support for pregnant and parenting women provided by skilled public health nurses in the home. |

Confidential women's health care that includes health screening, education, and birth control. Take charge program information. |

Free breast and cervical health screening, including mammogram, provided for low income women over the age of 40. Program also provides individual and community based education and assistance with necessary follow-up services. |

Identification, outbreak investigation, and monitoring of communicable disease such as tuberculosis, hepatitis, measles, food-borne illnesses, etc. |

|
Confidential diagnosis, treatment, and follow-up of sexually transmitted diseases such as herpes, gonorrhea, etc. |

Confidential and anonymous counseling/testing for diagnosis of HIV/AID (blood-borne pathogen screening, testing, counseling and follow-up for individuals who have experienced a significant exposure to blood). |

Comprehensive health and developmental assessment, anticipatory guidance, health and parenting education/support and resource coordination provided in the clinic or home setting. |

Consultation and direct nursing services provided to county school districts. |
|
|
Development and health status assessment, resource coordination and referral for children with special needs. |

Screening, diagnosis and referral for follow-up services. Preventative care and oral health education provided for individuals and groups. |

Literature, films and speakers available to speak with community groups on a variety of health related topics. |

Retailer and youth targeted education and prevention program. |

Confidential assessment, resource identification/coordination, and support for individuals and families affected by HIV/AIDS. |

Health and social services resource information and referral provided for the general public. |

|
Preparation and information regarding birth and death records. |

Regular collection, analysis, and sharing of information about health conditions, risks, and resources. |

Utilizing information identified through assessment to work with other health professionals, community members, organizations, and other interested parties to identify health priorities. |

Working with the community and other providers to assure that the policies and strategies developed to address priority health problems are carried out in an effective and efficient manner. |
